To schedule an appointment, call (717) 691-9214. Except for emergency situations, all in-home euthanasia services must be scheduled 24 hours in advance of the appointment.
Prior to the administration of any medication, you will be required to sign a release form to proceed with the euthanasia procedure. This is a standard form that is required by the state. Click here to download and print the form.
